Terms of Service

Welcome to Checkup LLC ("Checkup", "we", "our", or "us"). These Terms of Service ("Terms") govern your access to and use of our web application ("App"), available atcheckupapp.org(the "Site"). By creating an account or using the App, you agree to these Terms.

1. Data Collection and Privacy

We collect and store the following data: first name, last name, email, password, phone number, mood, productivity, stress, and fatigue. We also share limited user data with third-party providers, such asClerk for authentication and Stripe for payment processing. Stripe only receives your user ID during the purchase process. All data is securely stored and encrypted to protect your privacy.

2. Subscription Terms

Our subscription model allows users to purchase one-month access to our services without auto-renewal. Future subscription tiers may also be offered without auto-renewal. You may cancel your subscription and request a refund within the first 7 days of purchase.

If you upgrade to a higher tier (e.g., Standard over Basic) while you still have time remaining on your current Basic subscription, the Basic tier will be immediately deactivated, and your new subscription will start immediately. You will lose all access to the Basic tier once the new subscription is active.

3. Account Security

To protect your account, you are required to use a strong password and enable two-factor authentication (2FA). You are responsible for keeping your account secure and notifying us of any unauthorized access.

4. Data Retention and Deletion

We retain your data indefinitely unless an organization requests deletion. When deletion is requested, we will verify the request within 30 days. Users can also request to delete their data via email, and organizations will be notified of a user’s withdrawal.

5. Limitation of Liability

Checkup LLC does not provide professional mental health services and is not liable for any damages related to the use of our App. Our platform is a tool for tracking mental health data, not a substitute for professional care. We are not liable for any indirect damages or consequences from using our services.

6. Dispute Resolution

All disputes shall be first resolved through mediation in Connecticut before further legal action is taken. Mediation costs will be borne by each party.
